YOUR 25-26 PHYSICAL EXAM WILL NEED TO BE AFTER JUNE 9th 2025

Clearance deadlines to ensure eligibility for tryouts each season.

Fall – July 18th     Winter – October 24th      Spring – January 23rd

1 – Download:

4 – Print & Turn In:

 ☐ Print your confirmation message, sign, and staple to your physical form & turn in.

All forms must be turned in to the main office (coaches may not accept) at the front desk or the no contact drop box.

Students must use only the 25-26 SDUSD forms…….Leave nothing blank!

You may not practice, compete, or receive a uniform until all forms have been completed. Each coach has access to the clearance database to confirm.
