Athletic Clearance for the 26-27 Seasons

YOUR 26-27 PHYSICAL EXAM WILL NEED TO BE (on/or) AFTER JUNE 8th 2026

 ☐ Print your confirmation message, sign, and staple to your physical form & turn in.

All forms must be turned in to the main office (coaches may not accept) at the front desk or the no contact drop box.

Students must use only the 26-27 SDUSD forms…….Leave nothing blank!

You may not practice, compete, or receive a uniform until all forms have been completed. Each coach has access to the clearance database to confirm.
